To refill your pet's medication, please either:

- Call us at (253) 839-4744 during normal business hours or,
- Send us an email message at info@CrestwoodAnHosp.com with the word REFILL in the subject line or,
- Sign into your pet's portal and purchase refills online to be shipped to your home directly

To serve you better and refill your medication faster, we'll need to know the following information:

• Pet owner's name
• Pet's name
• Medication name and quantity needed
• Telephone number where the pet owner can be reached (in case we have questions)

We'll call you when your refill is ready for pick up.
